Rates and Policies
My rates include the time spent with you during your visit, as well as the time spent researching your case and preparing reports for your physician. Also included are referrals, answers to short questions by phone or e-mail, and providing coded receipts for insurance reimbursement. Please contact me to discuss my rates and services.
Payment is due in full at the time services are rendered and may be made by cash or personal check.
Phone Calls and E-Mails
You may phone or e-mail me when necessary and I will respond as soon as possible, generally within 24 hours. Phone and e-mail consultations are limited to 10 minutes without charge. I am generally not available on the weekends or past 7pm in the evening. If you have a medical emergency during these times, please call 911 or seek urgent care. Otherwise, please leave a message for me and I will get back to you as soon as possible.
Out of consideration for all of my patient’s health and schedules, I ask that you be on time for your appointment. To this end, if you are late, your treatment time may be reduced by the amount of time that you are late in order to ensure that I am on time for my next patient. Additionally, if you are more than 20 minutes late for your appointment, your appointment will be cancelled and you will be charged the full payment for your session.
If you need to cancel an appointment, please give me the courtesy of as much notice as possible, with a minimum of 24 hours (1 full day) notice. I will do my best to reschedule your treatment for another time in the same week. If you fail to show up for your appointment, do not give 24 hours notice, or are more than 20 minutes late, you will be charged the full amount for your missed appointment.
